What Ongoing Management Looks Like

Managing a chronic condition is not a one-time visit — it's a continuous relationship. Here's how we structure ongoing care:

Regular Check-Ins
We schedule follow-up appointments based on the stability and complexity of your condition — anywhere from every few months for well-controlled conditions to more frequently when adjustments are needed. We never let significant time go by without knowing how you're doing.

Lab Monitoring
Most chronic conditions require routine lab work to track progress and catch changes early. We manage your lab schedule, review results with you in the context of your full health picture, and make medication or lifestyle adjustments based on what we see.

Medication Management
Many patients with chronic conditions take multiple medications. We review your full medication list regularly, monitor for interactions and side effects, and work to simplify your regimen when possible. We also help you understand what each medication is doing and why it matters.

Lifestyle Guidance
Medication alone rarely tells the whole story. Nutrition, physical activity, sleep, and stress management all play real roles in how well chronic conditions are controlled. We incorporate practical lifestyle guidance into every chronic disease conversation, not just a pamphlet handed to you at checkout.

Specialist Coordination
When your condition requires specialist input — cardiology, endocrinology, nephrology, pulmonology — we coordinate referrals and stay in the loop on what those providers recommend, so your care doesn't get siloed.

Yes — and this is one of the most important things we can offer. Specialists manage one part of your health; we manage all of it. We review how your chronic condition interacts with your other health concerns, your medications, and your overall wellbeing in ways that specialists often can't focus on. Coordinated care leads to better outcomes.

That's exactly when we dig deeper. We'll review what's working and what isn't, look at whether adherence, side effects, or other health factors are playing a role, and adjust the plan. Sometimes a referral to a specialist or a change in medications makes a meaningful difference — we'll work through the options with you.

Many chronic disease follow-up appointments can be done via telehealth, especially for stable conditions. Some visits — those requiring a physical exam or in-office labs — will need to be in person. We'll help you figure out which type of visit makes the most sense at each point in your care.

Our patient portal gives you a direct line to our team for questions between visits. For patients enrolled in our Chronic Care Management program, you also have access to ongoing care coordination support between appointments. And if something changes, we want to hear about it — don't wait for your next scheduled visit if something feels off.
