4. Appointment scheduling

4.1 Scheduling policy

The Website provides an online appointment scheduling tool for the convenience of our patients. Submitting an appointment request does not guarantee that an appointment has been confirmed. Appointments are confirmed only upon receipt of a confirmation email or direct communication from our office.

4.2 Cancellation and no-show policy

We require a minimum of 24 hours’ notice for cancellation or rescheduling of appointments. Failure to cancel within this window, or failure to attend a scheduled appointment (“no-show”), may result in:

• A cancellation or no-show fee, as disclosed at the time of scheduling
• Restrictions on future online scheduling
• A requirement to pre-pay for future appointments

5.1 Portal access

5.2 Account credentials

You are responsible for maintaining the confidentiality of your Portal login credentials (username and password). You agree not to share your credentials with any third party. You must notify Gilbert Center for Family Medicine immediately at the contact information in Section 19 if you suspect unauthorized access to your account.

Gilbert Center for Family Medicine is not liable for any loss or damage arising from unauthorized use of your account credentials when such use results from your failure to maintain their confidentiality.

5.3 Portal availability

We strive to maintain Portal availability 24 hours a day, 7 days a week; however, we do not guarantee uninterrupted access. The Portal may be temporarily unavailable due to scheduled maintenance, system upgrades, or circumstances beyond our control. We will endeavor to provide advance notice of planned downtime where practicable.

5.4 Portal communications

Messages sent through the Portal are not monitored in real time and should not be used for urgent or emergent medical matters. For urgent medical concerns, contact our office directly by phone or, in an emergency, call 9-1-1.

6. Payment terms

6.1 Fees and charges

Fees for services are provided at the time of scheduling or upon request from our billing department. Gilbert Center for Family Medicine reserves the right to modify its fee schedule at any time. Changes to fees will not affect services already scheduled and confirmed at prior rates.

6.2 Online payments

The Website accepts online payments for copayments, balances due, and other charges. By submitting a payment, you authorize Prime Medical Group, LLC dba Gilbert Center for Family Medicine to charge the designated payment method for the stated amount. All transactions are processed through a third-party payment processor using industry-standard encryption (TLS/SSL).

You represent that you are the authorized holder of the payment method used and that the payment information you provide is accurate and complete.

6.3 Insurance

Prime Medical Group, LLC dba Gilbert Center for Family Medicine may verify your insurance coverage as a courtesy; however, the final determination of coverage and benefits is made solely by your insurance carrier. You are ultimately responsible for all charges not covered by your insurance. We encourage you to confirm your benefits with your insurance carrier prior to your appointment.

6.4 Refund policy

Refund requests are reviewed on a case-by-case basis and must be submitted in writing to our billing department within 30 days of the date of service. Approved refunds will be issued to the original payment method within 10 business days. Pre-paid amounts for services not rendered due to Prime Medical Group, LLC dba Gilbert Center for Family Medicine cancellation will be refunded in full.

6.5 Collections

Accounts that remain delinquent may be referred to a third-party collection agency. In accordance with A.R.S. § 12-341, the prevailing party in any legal action arising from non-payment may be entitled to recover reasonable attorneys’ fees.
