Remote Patient Monitoring (RPM) is a way for us to stay connected to that data and act on it, without requiring you to come in every time something needs attention.

At Gilbert Center, we use RPM as an extension of your care — giving you the tools to track key health measurements at home and giving us the real-time visibility to catch problems early and keep your treatment on track.

How Remote Patient Monitoring Works

Get Set Up
If RPM is a good fit for you, we'll provide or arrange a monitoring device — such as a blood pressure cuff, glucometer, pulse oximeter, or weight scale — and walk you through how to use it. The device transmits your readings automatically; you don't have to remember to log or report them.

Take Your Readings at Home
You use the device as directed — usually once or twice a day, or as often as your condition requires. The readings are sent securely to our care team in real time. Most patients find this takes less than two minutes out of their day.

We Review and Respond
Our care team reviews your data regularly. If your readings look stable, we note it. If something goes outside your target range — or a pattern emerges that concerns us — we reach out to you directly. That might mean a phone call, a message through the patient portal, or a recommendation to come in.

One of the biggest benefits of RPM is that it gives us the information to make smarter, faster adjustments. Instead of waiting for your next appointment to find out your blood pressure has been running high for weeks, we can catch it early — and act on it before it becomes a bigger problem.

What We Monitor

Blood Pressure
For patients with hypertension or cardiovascular disease, daily blood pressure readings at home give us a far more accurate picture than occasional in-office measurements. White coat hypertension (elevated readings in the office that normalize at home) is real — and so is the reverse. RPM helps us see what's actually happening.

Blood Glucose
For patients with diabetes or prediabetes, continuous or regular glucose monitoring helps us understand how your levels fluctuate throughout the day in response to meals, activity, stress, and medications. This data shapes more precise treatment decisions.

Oxygen Saturation and Heart Rate
Patients with COPD, asthma, heart failure, or a history of COVID-19 complications may benefit from pulse oximetry monitoring. A drop in oxygen saturation can signal an exacerbation before it becomes an emergency.

Weight
For patients with heart failure or other conditions where fluid retention is a concern, daily weight monitoring can detect changes of even a pound or two that might signal a problem — often days before symptoms appear.

No — the devices are designed to be simple. Most require nothing more than wrapping a cuff around your arm, pressing a button, or stepping on a scale. The data transmits automatically. If you ever have trouble with the device, our team is available to help.

That's the whole point. If we see a reading — or a pattern of readings — that warrants attention, we contact you promptly. Depending on what we see, we may adjust your medication, schedule a visit, or in urgent situations, direct you to immediate care. You're not just sending data into a void.

Medicare covers RPM for patients with chronic conditions who meet qualifying criteria. Many commercial insurance plans cover it as well. Our team will verify your coverage before you enroll so there are no surprises.

No — RPM supplements your in-person care, it doesn't replace it. You'll still have scheduled visits with your provider. What RPM does is make those visits more informed and productive, because we come in with real data rather than relying solely on how you've felt over the past few months.
